Waupun Correctional and Dodge Correctional Prison Records
Unfortunately, we have no prison records here
at the Waupun Public Library.
In September of 2005 we wrote to the Wisconsin
Department of Corrections in Madison, WI regarding the records of
incarcerated or previously incarcerated individuals. We
received a response back in November of 2005. This is the
information received:
"We do have records located here in Madison, WI, these are for offenders who
have been released from a correctional center or institution. We also have
records at each institution, which are records for currently incarcerated
persons. The proper procedure to request copies is the individual needs to
send a written request asking for specific information. They will also need
to read, fill out, and sign an 1163 form, which is the Authorization to
Disclose Non-Health information or 1163A form, which is the Authorization
for Use and Disclosure of Protected Health Information."
Later on in the letter, it is mentioned that they are only required to keep
5 years of records; however, we
do not know if this is all records or just certain ones.
Also mentioned was that a random 10% of the
records are given to the Wisconsin Historical Society, so if the Wisconsin
Department of Corrections can't help, you could contact the Wisconsin
Historical Society.
The forms mentioned above can be found by doing a Google
search for them (put in "1163 form Wisconsin DOC"). The address to send the

If you receive a response that they do not have any records, there may be
records at the Wisconsin Historical Society. Visit their website at
www.wisconsinhistory.org. Click on Library & Archives at the right. Then
click on Contact Us under the Information Services area at the right. Then
click on Library Reference & Information Services under the section "The
Library". Fill out the Reference Form and ask about how to access the
Wisconsin Department of Corrections records. You might want to be specific
about the type of records you are looking for.
We recently learned that there are old records at
Waupun Correctional; however, we do not know what
dates are covered. You may wish to write to the warden at Waupun
Correctional:

You may want to send these requests at the same time because it may take a
while to receive an answer from any institution.
